The Signup Forms page contains a list of saved signup forms for merchants boarded through your account. A signup form is a pre-made, customizable template that allows you to board new merchants.
To access the Signup Forms page, select Signup Forms under Forms in the left navigation panel.

Signup Forms Features
The Signup Forms page displays a table of your previously saved signup forms for specific merchants. The table contains the following information:
ID: The Payrix Pro-generated ID associated with the signup form.
Name: The name of the signup form.
Code: The autogenerated code associated with the signup form.
Created: The date and time that the signup form was created.
Signup Forms Actions
The Signup Forms page supports accessing more information and copying a link.
To access more information about a saved merchant signup form, select any information in the table row to open the Merchant Signup Form Details page.
To copy a signup form link:
Click the three-dot icon on the right side of the table.
Select Copy Link to copy the link to the signup form to your clipboard.
You can email the link or embed it in your website to reuse the previously saved form.
Merchant Signup Form Details Page
The Merchant Signup Form Details page contains the details of a previously saved signup form used to board a Merchant to the portal.
To access the Merchant Signup Form Details page:
Select Signup Forms in the Forms category of the left navigation panel.
Locate the saved form in the Signup Forms table.
Select any information in the table row to open the Merchant Signup Form Details page.
Code Submissions Tab
The Code Submission section displays information about any previously submitted forms, including the following details:
Created: The date that the signup form was submitted.
Status: The status of the signup form submission.
Entity: The entity that submitted the form.
IP Address: The IP address associated with the submission.
User Agent: The HTTP header that identifies the user responsible for submitting the form.
Legal Name: The legal name of the user who submitted the form.
DBA: The name that the user who submitted the form is doing business as.
First Name: The first name of the user who submitted the form.
Last Name: The last name of the user who submitted the form.
Phone: The phone number of the user who submitted the form.
Email: The email address of the user who submitted the form.
Errors: If the form contains errors, the field that is causing the error and the error description.
